The following information does not apply to Texas forensic records. Forensic records are released as permitted by the court or the retaining attorney. Requests for Texas clinical treatment records should be made in writing by the client (or parent/guardian), accompanied by proof of identity (e.g., driver’s license). The written request should be detailed and include the address to which records should be faxed or mailed. You will be notified of the cost of providing records consistent with Texas law. The records will be provided within 14 days of payment of the records fee. Clinical record requests should be mailed to Davidson Forensic Group, 800 W. Main, STE 1460, Boise, ID 83702.
